You ("Subscriber") have requested a subscription to access MedCollect ® under the terms set forth below. Use of MedCollect through the identification number issued pursuant to this Subscription Agreement is subject to the terms and conditions set forth in this document in the form originally provided to you by Legal Software Systems Corporation, as well as the General Terms and Conditions, which are incorporated herein by reference and which may be reviewed at the following General Terms and Conditions, all of which, collectively, represent the entire agreement for access to and use of MedCollect services.

::USER

This subscription is only available to attorneys and companies in the traditional practice of law and only permits the use of the MedCollect service by the attorneys (partners, associates, and staff attorneys) of the firm or company named below and non-attorney employees of the firm or company in the course of performing work solely on behalf of those attorneys. Subscriber shall only use MedCollect after acquiring a Password from Legal Software Systems Corporation for each User.
